What is Care-a-Van
Care-a-Van is a transportation service provided by Community Memorial Hospital. Care-a-Van will transport patients to and from physician offices, within a 17 mile radius of Hicksville, and to Community Memorial Hospital.

This service was made possible through the generous donations of the Hicksville Rotary, CMH Foundation, and CMH Auxiliary.

How do I schedule a ride?
Schedule your medical appointment, and then call Care-a-Van at (419) 542-5595. We will need the following information:

  • Patient name, address, and age (if a minor child)
  • Driving directions
  • Date and time of medical appointment
  • Destination (physician or CMH)
  • Number of people accompanying the patient
  • Whether or not the patient uses a wheelchair
    (wheelchair must be provided by the patient)

If you need assistance getting from the house to the van, please let us know when you schedule your ride.

Fees
Care-a-Van Services are provided free of charge by Community Memorial Hospital. If you wish to make a donation, please forward it directly to the Hospital Foundation.

Service Area
Care-a-Van provides patient transportation within a 17 mile radius of Community Memorial Hospital. This radius circles north through Edgerton, east to include Sherwood, Mark Center, and Cecil, south to just beyond Antwerp and west to include Harlan, Grabill, and St. Joe, Indiana.

I use a wheelchair, can you accommodate my needs?
The van is wheelchair accessible. We have the ability to transport one wheelchair in addition to three other passengers. You need only be in the wheelchair and ready to go when our driver arrives.

My physician ordered a prescription, can you pick it up?
If your physician calls a prescription to the Hicksville Pharmacy, our driver would be happy to pick it up. We must have written permission and payment from the patient.

Your Safety
Van Equipment: Our van is equipped with one child safety seat, safety straps for securing a wheelchair, seat belts, fire extinguisher, first aid kit, and other supplies that may be necessary in an emergency situation.

Our drivers are trained to perform the following:

  • CPR
  • Basic First Aid
  • Assist patients with decreased mobility to and from the van
  • Secure passengers prior to engaging the vehicle
  • Secure the wheelchair

Drivers are NOT trained to:

  • Assist patients with dressing
  • Transfer patients to and from the wheelchair
