for skilled, short-term rehabilitative services where patients convalesce
in a comfortable, friendly, caring atmosphere close to home and loved
ones.
Our caring, professional staff of physicians, nurses, therapists, radiology and lab technologists are focused on placing the
needs and comfort of our patients in the forefront.
Our interdisciplinary care team meets weekly to address each patient's
needs, while creating realistic goals that are met through the use of
therapeutic exercise, modalities, manual therapy techniques, and patient
and family education. Whether recovering from a stroke or adapting to
physical limitation following an injury, our team is dedicated to working

& throat), obstetric/gynecology and orthopedist. In an ongoing effort to provide updated and quality
services to persons throughout our service area, we have acquired the

Radiology/Imaging Department at Community Memorial Hospital provides a
comprehensive range of high-tech services including general X-ray,
fluoroscopy, CT scans, ultrasound, 3D/4D ultrasound, mammography, and
Open MRI service. The
radiology/imaging staff is committed to preserving patient privacy and
dignity with the utmost professionalism.

Diabetes Education
While the number of people with diabetes continues to
increase, so does the understanding of the disease. At
Community Memorial Hospital, we want to pass that knowledge on
to you. Our Diabetes Education Program has been developed
to equip you to manage your diabetes through diet, exercise,
medication and blood glucose monitoring. Our dietitian,
nurse, and pharmacist can answer many of the questions you may
have about your diagnosis, including:
- How often should I check my blood sugar?
- How does food affect my blood sugar?
- Do I have to give up sweets?
- How do I treat a low blood sugar?
- What are the side effects of my diabetes medication?
- What is a healthy blood sugar level?
When you enroll in CMH's Diabetes Education program, you can
expect to receive up to 10 hours of diabetes education and
training. This may include one-on-one training for insulin
or other needs, as indicated by your physician. All
participants will be provided with a diabetes informational
manual, product samples and a free glucose meter while supplies
last.

Weight Management
Community Memorial Hospital is dedicated to the well-being of
the community and strongly advocates weight loss for those at
risk of developing health complications due to their weight.
In order to help you reach your goals, CMH is pleased to offer a
comprehensive 22-week weight management program.
Comprised of a combination of group classes and individual
consultations, CMH Weight Management aims to help participants
make healthy lifestyle changes to lose weight in a consistent,
permanent manner. Whether you have a lot or a little to
lose, our experienced weight management team is here to help you
along the way.
