Founded in 1998, the CMH Foundation is a community-based, tax-exempt organization created to provide donors a way to contribute to the continued growth and enhancement of health care services at Community Memorial Hospital in Hicksville, Ohio.
Funds and their purposes are listed below:
To be used where needed most as directed by the CMH Foundation Board of Directors.
To provide scholarships for students pursuing a career in a medical field.
A permanent capital fund that will support the mission of Community Memorial Hospital forever. The principle is not accessible, but the interest is used to benefit Community Memorial Hospital.
Funds are used to make improvements to hospital and clinic properties, and to purchase equipment.
Ways To Give
A contribution in memory of a deceased friend or relative, or in honor of a special person, doctor, or hospital employee is an especially meaningful tribute.
Planned giving provides a method for donors to make gifts to Community Memorial Hospital and also enjoy favorable financial, tax-saving, and income consequences.
You can make a gift of cash, stocks, bonds, real estate, or other assets to the CMH Foundation. Your charitable gift qualifies for tax advantages under federal law.
Bequest by Will
You can designate a portion of your estate to the CMH Foundation and, in some cases, receive a substantial reduction in federal gift and estate taxes.
You can name the CMH Foundation as a direct or contingency beneficiary of all or a portion of your retirement funds, IRAs, life insurance policies, and other assets.
Annuities and Trusts
A variety of annuity and trust methods can be customized to accomplish your charitable and personal goals. Charitable Gift Annuities, Charitable Remainder Trusts, and Charitable Lead Trusts are three opportunities you may want to discuss with your professional advisor.

Past Projects of the CMH Foundation
The CMH Foundation raised over $600,000 for the new hospital and the Aquatic Therapy Pool projects, and in early 2012, announced a campaign to raise $250,000 to relocate and enlarge the Respiratory and Laboratory Departments.
